ABSTRACT

Objective:To investigate the clinical value of high-frequency ultrasound combined with virtual touch tissue imaging and quantification in the assessment of limb muscle tension after stroke in patients.Methods:A total of 31 patients with stroke who received treatment in Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to May 2022 and 41 healthy controls who concurrently underwent physical examination in the same hospital were included in this study. Before rehabilitation treatment, the shear wave velocity of the main muscle groups of the limbs was measured using virtual touch tissue imaging and quantification in all participants. The shear wave velocity of the main muscle groups was compared between the affected and healthy sides of patients between two sides of patients. The patient's muscle tension was evaluated using the modified Ashworth Scale. The shear wave velocity of the affected muscle groups in patients was correlated with the modified Ashworth Scale score.Results:There were no significant differences in the shear wave velocities of the main muscle groups of upper (biceps, flexor digitorum sublimis, flexor digitorum profundus) and lower [medial head of the gastrocnemius muscle, lateral head of the gastrocnemius muscle] limbs between the left [(2.46 ± 0.26) m/s, (2.81 ± 0.50) m/s, (2.96 ± 0.31) m/s, (2.49 ± 0.44) m/s, (2.21 ± 0.20) m/s] and right [(2.42 ± 0.29) m/s, (2.80 ± 0.47) m/s, (3.02 ± 0.36) m/s, (2.54 ± 0.37) m/s, (2.18 ± 0.17) m/s] sides in healthy controls ( t = 0.78, 0.04, 0.83, 0.58, 1.15, P = 0.435, 0.967, 0.405, 0.558, 0.216). The shear wave velocities of the main muscle groups of upper [flexor digitorum sublimis (3.74 ± 0.67) m/s, flexor digitorum profundus (3.64 ± 0.60) m/s), biceps (3.63 ± 0.64) m/s] and lower [medial head of the gastrocnemius muscle (3.28 ± 0.61) m/s, lateral head of the gastrocnemius muscle (2.90 ± 0.37) m/s] limbs on the affected side in patients with stroke were significantly higher than (2.56 ± 0.40) m/s, (2.67 ± 0.38) m/s, (2.78 ± 0.41) m/s, (2.30 ± 0.21) m/s, (2.25 ± 0.23) m/s on the healthy side ( t = 11.81, 8.21, 8.75, 8.91, 10.43, all P < 0.001). The shear wave velocities of the main muscle groups of the upper (flexor digitorum sublimis, flexor digitorum profundus, and biceps) and lower (medial head of the gastrocnemius muscle and lateral head of the gastrocnemius muscle) limbs were positively correlated with the modified Ashworth Scale score ( r = 0.77, 0.70, 0.72, 0.74, 0.78, P = 0.007, 0.029, 0.021, 0.016, 0.001). Conclusion:Monitoring the shear wave velocities of the main muscle groups of the upper and lower limbs using high-frequency ultrasound combined with virtual touch tissue imaging and quantification can effectively reflect the change in limb muscle tension of patients with stroke, which is highly valuable for evaluating rehabilitation efficacy and prognosis in patients with stroke.

ABSTRACT

Background:Ultrasound?guided (USG) radial artery cannulation against the standard palpation technique increases the first attempt rate in both pediatric and adult patients. The objective of this study was to evaluate the benefits of USG versus the palpation technique in improving the first attempt rate in elderly patients. Methods: The patients over 65 years of age were randomized to the USG or Palpation group. The radial artery identification in the USG group was performed with the aid of the Sonimage HS 1. In the Palpation group, the radial artery was identified by manual palpation. The operators were cardiothoracic anesthesiologists. Overall success was defined as cannulation completed within 10 min. Results: Eighty patients (40 in each group) were recruited. The respective first attempt and overall success rate for the USG group were similar to the Palpation group (P > 0.999 and P = 0.732). The time to the first attempt and overall success were also similar (P = 0.075 and P = 0.636). The number of attempts, number of catheters used, and failure rates were similar between the groups (P = 0.935, P = 0.938, and P = 0.723). The number of successful cannulations within 10 min was similar for both the groups as categorized by the radial artery diameter (P = 0.169). Conclusions: The USG did not increase the first attempt or overall success rate of radial artery cannulation in the elderly patients undergoing cardiothoracic surgery. The time to first attempt and overall success were similar between both the groups. The number of attempts and number of catheters used were similar between both groups.

ABSTRACT

Objective:To explore the application effects of standardized patient (SP) and breast visual palpation simulation system on clinical probation of breast surgery for medical students.Methods:A total of 110 students were randomly divided into experimental group and control group, and each group was further divided into 5 subgroups. In the experimental group, SP scripts of five common diseases in breast surgery department were designed according to the syllabus before probation, and the teachers were trained. During the probation, we first talked about the characteristics of disease diagnosis and treatment, and then the five subgroups collected the history of the disease according to the SP of different diseases mentioned above, and checked the corresponding lesions on the breast visual palpation simulation system. In the physical examination, SP responded to the students' questions. The control group also talked about the above-mentioned five diseases during the probation, and then the five subgroups carried out history collection and physical examination for specific patients in the hospital. Finally, the teaching effects were analyzed through medical record writing, theoretical examination, physical examination and student evaluation. SPSS 26.0 was used for chi-square test (or Fisher exact test) and Mann-Whitney U test. Results:There was no significant difference in the theoretical examination between the two groups. It was demonstrated that the correct rate of experimental group was significantly higher than that of the control group in medical history collection, especially in the main symptoms, incentives, aggravation and remission factors, accompanying and differential symptoms and so on. And the positive results of breast, nipple and axillary lymph node palpation in the experimental group were significantly higher than those in the control group through the physical examination operation of breast visual palpation simulation system. The analysis of students' evaluation showed that the teaching efficiency of the experimental group increased, and the students' satisfaction was improved (satisfaction rate: 92.73% vs. 76.36%).Conclusion:The application of SP and breast visual palpation simulation system in breast surgery probation teaching can not only protect the privacy of clinical patients, but also improve the teaching effects, which can be popularized in the clinical teaching of breast surgery.

ABSTRACT

Objective:To analyze the factors influencing the difference in location of cricothyroid membrane between traditional surface palpation and ultrasonic biplane.Methods:One hundred and seven subjects of both sexes, aged 18 yr, underwent neck ultrasound examination in our hospital from August 2021 to December 2021, were selected.The structure of cervical airway was observed by ultrasound.The subjects in whom the structure of cricothyroid membrane was clearly shown were selected.The middle point of cricothyroid membrane was located by otorhinolaryngologist using surface palpation method and by ultrasonic doctor using ultrasonic biplane method.The distance between two positioning points was measured by a ruler.The subjects were divided into accurate group (distance between two points≤3 mm) and difference group (distance between two points>3 mm). The distance between cricoid cartilage and thyroid cartilage (spacing of cricoid cartilage and thyroid cartilage) and distance between the midpoint of cricothyroid membrane and skin (spacing of cricothyroid membrane and skin) were measured by ultrasound.The neck length and the maximum submaxillary distance in head up were measured by a ruler.The factors influencing the difference in location between the two methods were analyzed by multivariate logistic regression.Results:One hundred and four subjects with clear cricoid cartilage and cricothyroid membrane under ultrasound were selected.There were significant differences in the ratio of gender, body mass index (BMI), cricothyroid membrane-skin distance, neck length, and maximum submandibular distance between the two groups ( P<0.05). Multivariate logistic analysis showed that female ( OR=9.091, P<0.001), BMI ( OR=11.214, P=0.001) and increased cricothyroid membrane-skin distance ( OR=5.649, P=0.015) were the factors influencing the difference in location between the two cricothyroid membrane localization methods. Conclusions:Female, obesity (BMI>28.0 kg/m 2) and increased distance between cricothyroid membrane and skin are the factors affecting the difference in location of cricothyroid membrane between ultrasonic biplane method and surface palpation method, and bedside ultrasound is recommended for location in those with the factors mentioned above.

Background@#Estimation of fetal weight through ultrasound or clinically, is important in the management of pregnant women. In low resource settings, where ultrasound is scarce, determination of the superior clinical method between Johnson’s rule and palpation method is of significant value.@*Objective@#The objective of this study was to determine the best clinical method in estimating fetal weight in term parturients in a tertiary government hospital. 140 term mothers with singleton pregnancies in cephalic presentation were included in this study.@*Methodology@#Fetal weight was estimated using both palpation method and Johnson’s rule and compared to the actual fetal weight. Effects of body mass index (BMI), cervical dilatation, and engagement on the accuracy of both methods were evaluated using one-way ANOVA and test of proportions. The accuracy of both methods were calculated by mean absolute error and bias. Bland-Altman analysis was used to see limits of agreement and the mean difference between estimated fetal weight to actual birthweight.@*Results@#Mean estimated fetal weight (EFW) was 2846.39 ± 427.29g by Johnson’s and 2904.29 ± 372.79g by palpation with a mean actual birthweight of 3028.30 ± 441.52g. Using paired t-test, no significant differences were found in EFW by the two methods and actual birthweight. Palpation had more estimates that differed from actual by < 100 grams at 41.43% compared to 16.43% for Johnson’s with p < 0.001. Lower bias (7.11%) was seen in palpation compared to Johnson’s (12.09%) and with more precise estimates.@*Conclusion@#Palpation method is more accurate and reliable than Johnson’s rule. Clinical palpation is easy, cost effective, simple and should be considered as a diagnostic tool for fetal weight estimation especially in rural areas. The effect modifiers are cervical dilation for palpation and engagement for Johnson’s. BMI has no effect in accuracy of estimates in both methods.

ABSTRACT

This research evaluated the effect of milking on the physical exam of the mammary gland (MG) and milk examination in Saanen goats. Six properties were selected, four farms were used after evaluation of criteria selection and distributed in two groups according to the milking method: manual or mechanical. The physical examination of MG was performed using the techniques of inspection and palpation, in addition to the examination of the milk by strip cup, California Mastitis Test, somatic cell count, and bacteriological exam. It was possible to detect an association between the milking method with the most of physical exam of the MG: inspection of the MG (p = 0.001), inspection of the teat (p = 0.002), palpation of the MG (p = 0.054) and palpation of the teat (p = 0.036). The machine milking had an association with the reduction of the volume of MG (OR = 6.00), reduction of the teat size (OR = 16.19), and firm consistency of mammary parenchyma (OR = 2.39). The use of machine milking represented a less risk for an increase of the volume of the MG volume (OR = 0.288) and the presence of cisternitis (OR = 0.490). The association between the SCC and the type of milking was also detected (p = 0.002). Machine milking was associated with changesin the physical exam of MG and milk examination compatible with traumatic lesions. This information indicates inappropriate use of milking equipment in the dairy goat's properties.(AU)


Esta pesquisa avaliou o efeito da ordenha sobre o exame físico da glândula mamária (GM) e do leite em cabras Saanen. Seis propriedades foram selecionadas, quatro fazendas foram utilizadas após avaliação da seleção dos critérios e distribuídas em dois grupos de acordo com o método de ordenha: manual ou mecânico. O exame físico da GM foi realizado utilizando as técnicas de inspeção e palpação, além do exame do leite pelo teste da caneca de fundo escuro, California Mastitis Test, contagem de células somáticas e exame bacteriológico. Foi possível detectar uma associação entre o método de ordenha com a maioria dos exames físicos da GM: inspeção da GM (p = 0,001), inspeção do teto (p = 0,002), palpação da GM (p = 0,054) e palpação do teto (p = 0,036). A ordenha mecânica associou-se com a redução do volume de GM (OR = 6,00), redução do tamanho do teto (OR = 16,19) e consistência firme do parênquima mamário (OR = 2,39). O uso da ordenha mecânica representou menor risco para o aumento do volume do volume da GM (OR = 0,288) e presença da cisternite (OR = 0,490). Também foi detectada a associação entre a CCS e o tipo de ordenha (p = 0,002). A ordenha mecânica foi associada com as alterações no exame físico de GM e do leite compatíveis com lesões traumáticas. Essas informações indicam o uso inadequado do equipamento de ordenha na criação de cabras leiteiras.(AU)

Resumen: ANTECEDENTES: El angiofibroma celular (neoplasia mesenquitamosa) es un tumor poco frecuente conformado por células fusiformes y estructuras vasculares. Los angiofibromas celulares son neoplasias pequeñas, delimitadas, que pueden aparecer en mujeres de 40 a 50 años. CASO CLÍNICO: Paciente de 15 años, sin antecedentes patológicos de importancia para el padecimiento actual, que acudió al servicio médico debido a la aparición de una masa vulvar de crecimiento rápido, de 1 año de evolución. En la exploración física se observó una tumoración vulvar de 10 x 4 cm, en forma de herradura y en el contorno de la vulva se visualizó otra tumoración dependiente de la anterior, en el clítoris, de 7 x 5 cm, que simulaba un falo; ambas estaban adheridas a los planos profundos, sin causar dolor a la palpación, móviles, circunscritas, de consistencia blanda, con crecimiento de vello corporal en posición androgénica. Se llevó a cabo la resección del tumor; el procedimiento se inició con el acceso al labio mayor, sin afectar el meato uretral y el clítoris; se observó aumento de la vascularidad y la tumoración se disecó en su totalidad. El reporte histopatológico fue de angiofibroma celular. La evolución de la paciente fue favorable. CONCLUSIÓN: El angiofibroma celular de la vulva es una neoplasia mesenquimatosa con características clínicas, morfológicas e inmunohistoquímicas propias. El caso aquí expuesto es el primero reportado en una paciente adolescente.


Abstract: BACKGROUND: Cellular angiofibroma is a very rare tumor, it was described as a mesenchymal neoplasm, mainly made up of two components: spindle cells and vascular structures. Cellular angiofibromas are small-delimited malignancies and characteristically occur in women between 40 to 50 years of age. CLINICAL CASE: A 15-year-old female patient, with no pathological history of importance for the current condition, went to the medical unit due to the presence of a 1-year evolution vulvar mass, which was growing rapidly, 10 x 4 cm vulvar mass, horseshoe-shaped bearing the contour of the vulva, another tumor dependent on the previous in the clitoris area (7 x 5 cm), simulating a phallus, fixed to deep planes, not painful on palpation, mobile, circumscribed, of soft consistency, with growth of body hair in an androgenic position. In the surgical event, a tumor resection was performed, beginning an approach by the external part of the majors labia, respecting the urethral meatus and clitoris, an increase in vascularity was observed, dissecting the tumor in its entirety. The histopathological report was of cellular angiofibroma. The evolution of the patient was favorable. CONCLUSION: The cellular angiofibroma of the vulva is a mesenchymal neoplasm with the own clinical, morphological and immunohistochemical characteristics. The case presented is the first identity developed in an adolescent patient.

ABSTRACT

The use of technology has increased tremendously, by means of more reliable, smaller, more accessible and specially more user-friendly devices, which provide a wider range of features, and promote significant benefits for the population and health professionals. It is in this context that monitors and apps for heart rate (HR) measurement have emerged. HR is a clinical vital sign of diagnostic and prognostic importance. In response to body movement, HR tends to increase, in a direct relationship with the intensity of exercise. HR was primarily measured by the count of arterial pulse, and recently, HR can be precisely measured by monitors, bracelets and smartphone apps capable to perform real-time measurements and storage of data. This paper aimed to make a brief and updated review on the theme, providing a broader view of advantages and limitations of these resources for HR measurement in exercise. HR monitors and apps use basically two types of technology, optical sensor (photoplethysmography) and electrical signal from the heart. In general, these devices have shown good accuracy in measuring HR and HR variability at rest, but there are differences between brands and models considering the type, mode and intensity of exercise. HR measurements by monitors and smartphone apps are simple, accessible and may help cardiologists in the monitoring of the intensity of aerobic exercise, focusing on health promotion and on primary and secondary prevention of cardiovascular diseases

@#Background: Endotracheal tube cuff (ETTc) inflation pressure is usually not regarded as an important aspect during intubation. In this study, we compared measuring ETTc pressure and pilot balloon palpation method in causing post-operative airway complications. Methods: Two hundred and ninety-two surgical patients requiring intubation were recruited into this prospective, double-blind, randomised controlled study. Group A patients had their ETTc initially inflated, checked by a cuff pressure gauge, recorded and then set to 25 cmH2O. Group B patients had their ETTc inflated using the pilot balloon palpation method. Patients were then followed up for post-operative sore throat, hoarseness and cough. Results: The overall incidence of post-operative sore throat was 39.0% versus 75.3% (P < 0.001), hoarseness 6.2% versus 15.1% (P < 0.05) and cough 7.5% versus 21.9% (P < 0.05) in Group A and B, respectively. Group A patients experienced a significant reduction in the incidence and severity of sore throat up to 24 h post-operatively (P < 0.001), hoarseness at the first hour (P = 0.004) and cough at first and 12 h post-operatively (P = 0.002). Conclusion: Adjusting the ETTc pressure to 25 cmH2O reduces post-operative sore throat, hoarseness and cough compared to pilot balloon palpation method.

ABSTRACT

PURPOSE: The purpose of this study was to assess the intra-rater and inter-rater reliability and validity of pelvic tilting angle measurements using a smart phone-based inclinometer (Clino) compared to a palpation meter (PALM) in the standing and sitting position. This study used an interchangeable method with Clino to measure the pelvic tilting angle in the standing and sitting positions. METHODS: Twenty healthy subjects were recruited. Measurements of the pelvic tilting angle in the standing and sitting positions were obtained by two examiners using the Clino and PALM. A resting session was conducted 10 minutes later to assess the intra and inter rater reliability. To assess validity of the measurement using Clino, a PALM was used as the gold standard. The intra-class correlation coefficient (ICC) was used to determine the intra and inter rater reliability of Clino and a PALM. To assess the validity, the Pearson correlation coefficients were used for two measurement techniques to measure the pelvic tilting angle in the standing and sitting positions. The statistical significance was set to α=0.05. RESULTS: Measurements of the pelvic tilt had high inter-rater reliability in the standing (ICC=0.82) and sitting (ICC=0.88) positions using Clino and intra-rater reliability in the standing (ICC=0.87) and sitting (ICC=0.91) positions using Clino. Measurements of the pelvic tilt had high validity by a comparison of PALM and Clino in the standing (r=0.83) and sitting (r=0.89) positions (p<0.05). CONCLUSION: The use of Clino can be recommended as a tool to replace the PALM and measure the pelvic tilt angle in the standing and sitting positions while maintaining the clinical reliability and validity.

ABSTRACT

PURPOSE: The purpose of this study was to investigate magnetic resonance imaging (MRI) and 3-dimensional transperineal ultrasound (3D-TPUS) features of pelvic floor dysfunction (PFD) in symptomatic women in correlation with digital palpation and to define cut-offs for hiatal dimensions predictive of muscle dysfunction. METHODS: This prospective study included 73 women with symptoms suggesting PFD. 3D-TPUS, MRI, and digital palpation of the levator ani muscle were performed in all patients. Levator hiatal antero-posterior (LHap) diameter and area (LH area) were measured at rest and at maximum muscle contraction. RESULTS: The reduction in LHap diameter and LH area during contraction was significantly less in women with underactive pelvic floor muscle contraction (UpfmC) than in those who had normal pelvic floor muscle contraction by digital palpation (P<0.001). Statistically significant positive correlations (P<0.001) were found between the Modified Oxford Score and 3D-TPUS and MRI regarding the reduction in the LHap diameter (r=0.80 and r=0.82, respectively) and LH area (r=0.60 and r=0.70, respectively). A reduction in LHap of <6.5% on 3D-TPUS and <7.6% on MRI predicted UpfmC with sensitivities of 46.2% and 82.7%, respectively. A reduction in LH area of <3.4% on 3D-TPUS and <3.8% on MRI predicted UpfmC with sensitivities of 75.0% and 88.5%, respectively. MRI was more sensitive in detecting levator avulsion (63.4%) than 3D-TPUS (27.1%). CONCLUSION: MRI and 3D-TPUS had strong positive correlations with findings on palpation, and at certain cut-offs for hiatal dimensions, they can be used as complementary and objective tools to improve the accuracy of diagnosis and management planning of PFD.

ABSTRACT

The "Mai Dong (bing) Fa" (pathogenesis of channel or meridian disorders) was proposed in the concept by Huangfu MI (from the Jin Dynasty), and was classified and elaborated according to the characteristics and comprehensive analysis of symptoms and signs collected by inspection, auscultation and olfaction, inquiry and pulse taking and palpation in his book Zhen Jiu Jia Yi Jing (A-B Classic of Acupuncture and Moxibustion). In-depth exploration on the academic origin of "pathogenesis of meridian disorders" may help us understand the connotation of meridian problems once again. Firstly, when the meridian falls in disorder, resulting in abnormal functional changes and then a series of symptoms and signs arose. Secondly, generally, not just one meridian falls in disorders, but more meridians are simultaneously involved, which needs to be differentiated through comprehensive analysis of the four diagnostic methods and meridian identification. Thirdly, based on the diagnosis of the disorders of the related meridians, appropriate acupoints are selected. Correct understanding about the "pathogenesis of meridian disorders" is definitely helpful to our observation and summarization of modern clinical diseases and theoretical sublimation, and also favorable to guide our clinical practice and clarification of meridian-collateral's implications.

ABSTRACT

Professor - has gradually improved the clinical application of channel palpation treatment based on his more than 50 years clinical practice, and has accumulated rich experience in acupuncture treatment of polymyalgia rheumatica. He believes that "wind, cold and dampness" are the external causes of the disease, physical factors, uncomfortable mood and uncontrolled diet are the internal causes. The meridian-collateral theory is utilized in the diagnosis and detection of the disorders of , and meridians, internal and external causes are solved by expelling the wind, warming the channel to eliminate the coldness, transforming the dampness to relieve pain and regulating the activity. Three cases of clinical application on polymyalgia rheumatica were included in this paper.

ABSTRACT

Abstract The postural evaluation software (SAPO) has been used as a valuable tool for the analysis of postural alterations, however, such a tool depends on palpation performed by the evaluator. The aim of the present study was to evaluate the inter-and intra-examiner reliability of experienced and inexperienced examiners in measurements derived from the palpation method, and its possible influence on measurements obtained by SAPO. Nine evaluators participated in the study, which were divided into three groups according to experience with respect to palpation (GI: inexperienced group, GSE: semi-experienced group and GE: experienced group). Each evaluator performed the postural analysis of 10 volunteers, which were photographed in orthostatic position, following the SAPO recommendations. The intra-examiner reliability analysis indicated that all three groups had higher, good and moderate mean correlation values for each examiner than low correlations. In the inter-examiner reliability assessment, GE and GSE groups exhibited higher correlation values. The intra-class correlation coefficient, in the single-measure analysis, presented correlation coefficients <0.70 in 9 of the variables analyzed, indicating non-significant correlation. It was concluded that the measures analyzed by SAPO have high intra-examiner reliability. However, regarding inter-examiner reliability, the group composed of inexperienced evaluators presented lower values, suggesting that the experience time may have influenced the inter-examiner reliability regarding postural evaluation.


Resumo O Software de avaliação postural (SAPO) vem sendo utilizado como uma ferramenta valiosa para a análise das alterações posturais, porém tal ferramenta depende da palpação realizada pelo examinador. O objetivo do presente estudo foi avaliar a fidedignidade inter e intraexaminadores experientes e inexperientes nas medidas derivadas do método de palpação, e sua possível influência sobre as medidas obtidas pelo SAPO. Participaram do estudo nove avaliadores, divididos em três grupos de acordo com a experiência com relação a palpação (GI: grupo inexperiente, GSE: grupo semi experiente e GE: grupo experiente). Cada avaliador realizou a análise postural de 10 voluntários, os quais foram fotografados em posição ortostática, seguindo as recomendações do SAPO. A análise da fidedignidade intraexaminadores indicou que os três grupos apresentaram valores de correlação em média mais altos, bons e moderados, do que correlações baixas. Na avaliação da fidedignidade interexaminadores os grupos GE e GSE exibiram valores de correlação mais altos. O coeficiente de correlação intraclasse, na análise de medida única, apresentou índices de correlação <0,70 em 9 das variáveis analisadas, indicando correlação não considerável. Concluiu-se que as medidas analisadas pelo SAPO possuem uma alta fidedignidade intraexaminadores. No entanto, em relação a fidedignidade interexaminadores o grupo composto por examinadores inexperientes com relação a palpação apresentou valores mais baixos, sugerindo que o tempo de experiência dos examinadores pode ter influenciado na fidedignidade interexaminadores da avaliação postural.

ABSTRACT

Objective To study the consistency between pathology and the single and combined diagnosis of negative breast cancer by ultrasonography and mammography.Methods 90 clinical case data of patients with palpable negative breast tumors and mammography revealed small calcifications were retrospective analyzed.All patients were received the high frequency ultrasound.The value of two methods and combined diagnosis in the diagnosis of breast cancer with palpation negative were compared,which the pathological biopsy was the gold standard.Results The pathological showed that there were 58 cases of benign tumors,32 cases of malignant tumors,10 cases of stage 0,18 cases of stage Ⅰ,4 cases of stage Ⅱ.The high frequency ultrasonography was consistent with pathological(Kappa =0.641).The AUC of diagnosiing the breast cancer was 0.775.The molybdenum target radiography was consistent with pathological (Kappa =0.725),and the AUC was 0.830.The high frequency ultrasonography combined with molybdenum target radiography was highly consistent with pathological(Kappa =0.879),and the AUC was 0.934.The accuracy of combined diagnosis was higher than that of high frequency ultrasound (P < 0.05) and molybdenum target radiography(P > 0.05).Conclusion The high frequency ultrasonography and molybdenum target radiography have advantages in the diagnosis of palpable negative breast cancer.The joint diagnosis helps to achieve complementary strengths,has high consistency with pathology,and help to improve the diagnostic accuracy.

Objective To explore the diagnosis value of X-ray breast imaging reporting and data system (BI-RADS) combined with wire-guided localization biopsy for breast microcalcifications in impalpable breast cancer.Methods 192 palpation negative patients with 205 microcalcification lesions were detected by mammography.All lesions were classified according to BI-RADS descriptors for calcification and were categorized by the BI-RADS.The patients with BI-RADS category 4a and above underwent X-ray positioning guide wire-guided biopsy and pathological diagnosis.Results In 205 microcalcification lesions,74 (36.1%) were malignant lesions,131 (63.9 %) were less than benign lesions.The positive predictive value of malignant breast lesions in clustered,segmental,regional linear branching calcifications were higher [83.3 % (5/6),100.0 % (11/11),100.0 % (1/1)],followed by clustered,linear,segmental,regional pleomorphic calcifications [55.9 % (38/68),50.0 % (1/2),40.0 % (8/20) and 33.3 % (4/12),respectively].The positive predictive values of malignant in linear branching calcifications and pleomorphic calcifications were significantly higher than those of coarse heterogeneous calcifications,amorphous or indistinct calcifications (x2 values were 34.44,51.87,16.71,29.86,all P < 0.05).The linear branching calcification had the highest possibility.The proportions of malignant lesions in four different types of glands were extremely dense 40.5 % (30/74),heterogeneously dense 39.2 % (29/74),scattered areas of fibroglandular density 10.8 % (8/74) and fat 9.5 % (7/74),respectively.Conclusions BI-RADS categorization for breast microcalcification lesions can improve the detection rate of impalpable breast cancer.Linear branching calcification has higher predictive value for malignant lesions.Dense breast is the risk factors of breast cancer,which should be attached great importance.

Summary Introduction: Digital palpation and manometry are methods that can provide information regarding maximum voluntary contraction (MVC) and endurance of the pelvic floor muscles (PFM), and a strong correlation between these variables can be expected. Objective: To investigate the correlation between MVC and endurance, measured by digital palpation and manometry. Method: Forty-two women, with mean age of 58.1 years (±10.2), and predominant symptoms of stress urinary incontinence (SUI), were included. Examination was firstly conducted by digital palpation and subsequently using a Peritron manometer. MVC was measured using a 0-5 score, based on the Oxford Grading Scale. Endurance was assessed based on the PERFECT scheme. Results: We found a significant positive correlation between the MVC measured by digital palpation and the peak manometric pressure (r=0.579, p<0.001), and between the measurements of the endurance by Peritron manometer and the PERFECT assessment scheme (r=0.559, P<0.001). Conclusion: Our results revealed a positive and significant correlation between the capacity and maintenance of PFM contraction using digital and manometer evaluations in women with predominant symptoms of SUI.


Resumo Introdução: a palpação digital e a manometria são métodos capazes de fornecer informações sobre contração voluntária máxima (CVM) e endurance da musculatura do assoalho pélvico (MAP), e pode-se esperar uma forte correlação entre essas variáveis. Objetivo: investigar a correlação entre CVM e endurance, avaliados por palpação digital e manometria. Método: incluíram-se 42 mulheres, com idade média de 58,1 anos (±10,2) e sintomas predominantes de incontinência urinária de esforço (IUE). Realizou-se primeiramente o exame digital, seguido pela manometria (Peritron®). Mensuraram-se a CVM de acordo com a escala de Oxford (0-5 pontos) e o endurance pelo esquema PERFECT. Resultados: encontrou-se correlação positiva entre CVM mensurada por palpação digital e pressão manométrica de pico (r=0,579; p<0,001), e entre as medições do endurance avaliado pelo Peritron e o esquema PERFECT (r=0,559; p<0,001). Conclusão: os resultados revelaram correlação positiva e significativa entre a capacidade e a manutenção de contração dos MAP por meio das avaliações digital e manométrica em mulheres com IUE.

Background & objectives: Biopsy of palpable breast masses can be performed manually by palpation guidance or under imaging guidance. Based on retrospective studies, image guided biopsy is considered more accurate than palpation guided breast biopsy; however, these techniques have not been compared prospectively. We conducted this prospective study to verify the superiority and determine the size of beneficial effect of image guided biopsy over palpation guided biopsy. Methods: Over a period of 18 months, 36 patients each with palpable breast masses were randomized into palpation guided and image guided breast biopsy arms. Ultrasound was used for image guidance in 33 patients and mammographic (stereotactic) guidance in three patients. All biopsies were performed using 14 gauge automated core biopsy needles. Inconclusive, suspicious or imaging-histologic discordant biopsies were repeated. Results: Malignancy was found in 30 of 36 women in palpation guided biopsy arm and 27 of 36 women in image guided biopsy arm. Palpation guided biopsy had sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of 46.7, 100, 100, 27.3 per cent, respectively, for diagnosing breast cancer. Nineteen of 36 women (52.8%) required repeat biopsy because of inadequate samples (7 of 19), suspicious findings (2 of 19) or imaging-histologic discordance (10 of 19). On repeat biopsy, malignancy was found in all cases of imaging-histologic discordance. Image guided biopsy had 96.3 per cent sensitivity and 100 per cent specificity. There was no case of inadequate sample or imaging-histologic discordance with image guided biopsy. Interpretation & conclusions: Our results showed that in palpable breast masses, image guided biopsy was superior to palpation guided biopsy in terms of sensitivity, false negative rate and repeat biopsy rates.

Objective:Ultrasound guided by Mammotome spiral cut (MMT) system and the traditional bard needle aspiration biopsy in the diagnosis of nonpalpable breast lesions.Methods:Chooses January 2014 to 2016 January in our hospital were 113 cases of breast ultrasound,mammography found and nonpalpable breast lesions were retrospectively analyzed,a total of 167 breast lesions,of which 80 cases (120 lesions in ultrasound guided by MMT rotary cutting biopsy (MMT group),33 cases (47 lesions) take bard biopsy (Bard group),with postoperative pathological results as the gold standard,comparison of the two methods diagnostic value.Results:MMT group biopsy success rate of 100% was significantly higher than that of Bard group 87.23%,the difference has statistical significance (P<0.05);MMT method biopsy for differential diagnosis sensitivity of benign and malignant nonpalpable breast lesions is 96.67%,and the specificity was 97.78%,misdiagnosis rate was 3.33%,and the misdiagnosis rate was 2.22%;bard biopsy to identify diagnostic sensitivity of benign and malignant nonpalpable breast lesions was 83.33%,the specificity was 92.22%,misdiagnosis rate was 16.67%,misdiagnosis rate was 7.78%.MMT biopsy complication rate was 10% significantly lower than the Bard group 30.30%,the difference was statistically significant (P < 0.05).Conclusion:Ultrasound guided MMT biopsy needle puncture biopsy with Bard has a higher diagnostic value for breast nonpalpable lesions.
